Japan Visa Application Requirements
Passport (6 months valid)
2.) Visa Application Form-A4 size bond paper
3.) Passport Photo (1 copy of 4.5 cm x 4.5 cm with white background)
4.) Birth Certificate (NSO-issued with receipt and valid within 1 year)
5.) Married Contract (if married) NSO-issued with receipt and valid within 1 year
6.) Bank Certificate and Bank Statement
7.) COE, Company ID, and LOA (FOR EMPLOYED); DTI, Business Permit, COR(FOR SELF-EMPLOYED)
8.) Income Tax Return (Form 2316 or 1701) photocopy
ADDITIONAL REQUIREMENTS :
= Notarized Affidavit of Support (IF SUPPORTED / SPONSORED BY ANOTHER PERSON)
= DSWD clearance for minor (BELOW 18 YEARS OLD, NOT TRAVELLING WITH PARENT)
= Birth Certificate and / or Marriage Contract (TO ESTABLISH THE RELATIONSHIP BETWEEN THE SPONSOR / ACCOMPANYING PERSON)
= School Certificate / Certificate of Enrollment with copy of school ID (FOR STUDENT)
= Proof of financial documents form the accompanying person / sponsor
